Prijava

Prijavu za kongres možete ispuniti ovdje. Također, prijaviti se možete i pomoću priloženog obrasca. Sve sudionike koji se neće prijaviti preko mreže ljubazno molimo da ispunjeni obrazac običnom poštom pošalju tajniku Kongresa. Nakon uplate kotizacije tajnik će Vam poslati potvrdu Vaše prijave. Molimo Vas da se predbilježite za svaki društveni događaj na kojem želite sudjelovati. Troškovi koktela dobrodošlice i svečane večere uključeni su u kotizaciju za sve sudionike i pridružene osobe. Stručne ekskurzije nisu uključene u kotizaciju.

Kotizacija

Sudionik(ica): 500,00 kn/nakon 15. lipnja 700,00 kn
Osoba u pratnji: 250,00 kn/nakon 15. lipnja 350,00 kn
Sudionici mlađi od 25 godina, nezaposleni i umirovljenici: 300,00 kn/nakon 15. lipnja 500,00 kn

Kotizacija za sudionike(ice) obuhvaća: kongresne materijale, knjigu sažetaka, čaj i kavu, koktel dobrodošlice, svečanu večeru, pristup svim kongresnim zbivanjima.
Kotizacija za pridružene osobe obuhvaća: koktel dobrodošlice i svečanu večeru.

Studenti ispod 25 godina starosti, nezaposleni i umirovljenici tajniku Kongresa moraju dostaviti dokument kojim dokazuju svoj status.

Plaćanje

Svi troškovi moraju biti unaprijed podmireni. Tek nakon zaprimanja potvrde o namirivanju troškova kotizacije tajnik će potvrditi Vašu registraciju, a sažetak će ući u knjigu Sažetaka. Kotizaciju možete uplatiti u Zagrebačkoj banci na žiro račun: 2360000-1101560443 uz poziv na broj 1885.

Otkazni rok registracije

Sudionici Kongresa moraju pismenim putem (fax ili e-mail) obavijestiti tajnika Kongresa o otkazivanju registracije. Otkazivanje će biti prihvaćeno do 15. srpnja 2003. nakon čega će troškovi umanjeni za 20% biti vraćeni sudioniku. Žalimo što nikakav povrat troškova neće biti moguć nakon 1. kolovoza.

Registration

You can register on-line . You can also use the registration form included with this announcement or download it. All participant not registering through the website are kindly requested to fulfill this form and send it to the Secretary of the Congress. A confirmation will be send upon receipt of payment. You will need to register for all social events you wish to attend. Note that the welcome cocktail and the gala evening are included in the registration fee for all participants and accompanying persons. Excursions are not included in the registration fee.

Registration fee

Participant: 65 Euro/95 Euro, after June 15
Accompanying person: 35 Euro/45 Euro, after June 15
Participants under age of 25, unemployed and retired persons: 40 Euro/65 Euro, after June 15

Registration fee for participants includes following: all conference material, abstract book, daily tea/coffee, the welcome reception, gala evening, admission to all conference sessions and entrance to the poster/exhibition area. Registration fee for accompanying persons includes following: the welcome reception and gala evening. Please note that the fee for accompanying persons does not include admission to the scientific sessions, coffee breaks nor entitle them to the conference documentation. Students under age of 25, unemployed and retired persons must submit proof of their status to the Secretary of the Congress.

Payments must be made in advance and your registration/abstract can only be accepted and handled further when payment has been received. Payments can be done at:

Cancellation of Registration

Notification must be done in writing (fax or e-mail) and sent to the Secretary of the Congress. Cancellation of registration will be accepted until July 15, 2003, up to which date the total amount will be refunded except for a 20% cancellation charge. We regret that no refund can be made for any cancellations received after August 1.
